An example of my intentions

I started writing down my aspirations in 1994. I used to call them goals, and some were shoot-the-moon ambitions, including serving as the Governor of Virginia someday. I struck out on that one and many others but hit a few home runs, including the founding of Planet Subaru. As I’ve satisfied some of my younger desires to ascend various summits, I’m now more interested in broader pursuits, such as improving myself as a person and increasing my ability to serve others. I began referring to my goals as intentions, thinking of them as life-long projects, rather than tasks to check off.

This document gives an example of many formats you can choose to set your annual intentions.
